Our Outreach Services program helps to fill many of the gaps faced by people with disabilities. Traditional funding streams limit the kinds of equipment and activities that can be purchased. Through the generous support of United Way of Mahaska County, Imagine the Possibilities is able to meet many of those needs, such as camper ships and special trips, Special Olympics, Challenger Little League Team and other special gifts and grants. Individuals served within our HCBS program must have primary diagnosis of Intellectual Disability, Brain Injury or Chronic Mental Illness. We provide SCL (Daily and hourly) and respite services to both children and adults. Our mission is “Individual Achievement.” We assist individuals to accomplish this by working closely with the individual receiving services, parents/guardian, Case Manager and other team members to coordinate services that focus on maximizing independence through skills including but not limited to household, community involvement, budgeting, cooking skills, health and safety, etc. [WPSM_FAQ id=1472]

ICF/ID (Intermediate Care Facility for People w/ Intellectual Disabilities)

Service Directory
Individuals served within our ICF/ID program all have a primary diagnosis of intellectual disabilities, and may require a high level of support. Beyond that, the individuals have a variety of secondary diagnoses including autism, cerebral palsy, communication deficit, behavioral disorders, as well as a wide variety of medical diagnoses. Several of the individuals require the use of a wheelchair for mobility and positioning. Many of the individuals served in the ICF/ID program are non-verbal. Individuals served in the ICF/ID program receive individualized program planning designed to enhance their abilities and gain independence in daily living skills. Day Habilitation Services are provided to individuals who would like to work on life-skill goals such as cooking, cleaning, socialization, community safety, etc. through a CARF-certified program called Community Integration.
