Beyond that, the individuals have a variety of secondary diagnoses including autism, cerebral palsy, communication deficit, behavioral disorders, as well as a wide variety of medical diagnoses. Several of the individuals require the use of a wheelchair for mobility and positioning. Many of the individuals served in the ICF/ID program are non-verbal.

Individuals served in the ICF/ID program receive individualized program planning designed to enhance their abilities and gain independence in daily living skills. There is trained staff on duty 24 hours per day, seven days a week.

ICF/ID Factsheet

ICF/ID stands for Intermediate Care Facility for people with Intellectual Disabilities. This group living environment is designed to meet the needs of individuals with moderate to severe/profound disabilities who require 24-hour supervision and structure. The ICF/ID provides intensive services designed to train individuals while addressing a variety of disability related situations. Services include skills training, activities, dietary management, health care and therapies.
An Interdisciplinary team (IDT) is developed for each person served. The IDT is comprised of the person receiving services, service providers, and family members. Through discussion, the team designs goals and objectives based upon the desires, abilities and specific needs of the person receiving services. These goals and objectives become the framework for services provided by staff in the program.
Imagine the Possibilities, Inc. own and operates one 12-bed licensed ICF/ID facility in Maquoketa Iowa. In addition The Agency own and operates 4 six bed licensed ICF/ID facilities in Dewitt, Iowa, 2 six bedroom homes in Corning, Iowa- 3 six bedroom homes in Creston, Iowa and an 8 bed facility in Oskaloosa Iowa.
Imagine the Possibilities employs an administrative level person with the responsibility of overseeing and coordinating the ICF/ID program. This individual supervises the direct support staff and participates in the delivery of services by coordinating IDT meetings, counseling, interagency coordination, and the writing and monitoring of the Individual Program Plan as outlined by the IDT.
The ICF/ID program is funded primarily by the Federal Medicaid Program with additional participation by the consumer and the State of Iowa. Consumers who are not eligible for Medicaid funding pay for services privately.
Eligible persons must present a developmental disability, including Mental Retardation, not pose a serious threat to self or others, be medically stable – does not require higher level of care, show favorable prognosis for positive outcomes with programming, and be approved for payment of fees.
The ICF/ID program can be accessed by either contacting Imagine the Possibilities directly or Mahaska County Community Services. Screening will be done by IMAGINE staff to determine the ability to provide the needed support. Funding must be identified and secured.

Corning and Creston

Imagine the Possibilities Inc. Western region owns and operates 5 six bedroom homes.  Two homes are located in Corning and three homes are located in Creston.   Individuals served at these locations all have a primary diagnosis of mental retardation, and may require a high level of support. Beyond that, the individuals have a variety of secondary diagnoses including autism, cerebral palsy, communication deficit, behavioral disorders, as well as a wide variety of medical diagnoses.
