123807Day Habilitation Services are provided to individuals who would like to work on life-skill goals such as cooking, cleaning, socialization, community safety, etc. through a CARF-accredited program called Community Integration.

Individuals have the opportunity to be active and access their community through communications and socialization activities.


Day Habilitation Factsheet

Day Habilitation services are services that assist or support the member in developing or maintaining life skills and community integration. Services must enable or enhance the member’s intellectual functioning, physical and emotional health and development, language and communication development, cognitive functioning, socialization and community integration, functional skill development, behavior management, responsibility and self-direction, daily living activities, self-advocacy skills or mobility.
An Interdisciplinary Team (IDT) is developed for each person served. The IDT is comprised of the individual receiving services, service providers, and family members. The individual may invite other parties that they deem important. Through discussion, the team designs goals and objectives based upon the desires, abilities and specific needs of the individual receiving services. These goals and objectives become the framework for the services provided.
Service schedules are based on the needs of the individual and need to be reflective of the goals and objectives that are developed by the IDT. Services are monitored by the Day Habilitation Coordinator who communicates with the individuals Case Manager and guardian. Day Habilitation is accredited by CARF for Community Integration Services.
Day Habilitation Services are funded primarily through the Federal Medicaid Program and the State of Iowa. Some individuals may be funded through county or private funding sources.
Individuals eligible for Day Habilitation services must have a diagnosis of Mental Retardation or Mental Illness and be at least 16 years of age.
Day Habilitation services are provided in a community setting.

Dewitt, Iowa and Maquoketa, Iowa

Imagine the Possibilities provides Day Habilitation services throughout the local communities of Dewitt and Maquoketa. Individuals have the choice to volunteer in their community and participate in leisure activities with assistance from staff. Imagine the Possibilities serves approximately 80 individuals in our Day Habilitation program.

Garnavillo, Iowa and Edgewood, Iowa

Imagine the Possibilities provides Day Habilitation services in the communities of Edgewood and Garnavillo, with individuals from Clayton, Delaware, and Fayette County. Individuals have the choice to participate in a variety of volunteer activities, community activities, and leisure activities with assistance from staff. Imagine the Possibilities serve approximately 65 individuals in our Day Habilitation program.
